The discharge papers are signed. A bag of belongings is packed. Everyone is relieved to be heading home.
Then the questions begin.

Coming home from the hospital is an important milestone, but the first few days can feel demanding. A thoughtful plan can make the transition more comfortable and manageable.
Before leaving the hospital, ask for written instructions and clarify anything that feels uncertain:
Have any medications changed?
When are follow-up appointments needed?
Are there activities that should be avoided or adjusted?
What changes should prompt a call?
Who should the family contact with questions?
Keep discharge information, medication lists, provider phone numbers, and appointment details together where everyone involved can find them. If you are returning home, consider which routines matter most to you and where support would feel useful. If you are supporting a family member, make sure their preferences and choices remain part of the plan, regardless of how much support they need.
Think beyond the medical instructions and picture what the first few days will require.
Clear walking paths, improve lighting, and place frequently used items within comfortable reach.
